  1. Read the information for each section below. Answer the questions with pull-down boxes by clicking on them and then clicking on your choice. To answer the text questions, click in the upper left corner of the answer area and type your answer.

  2. Once you have completed the survey, click the Submit Your Registration button.

3. Driver

Would you be willing to be a driver of one of the COMET minivans during the course?

If so, you will need to sign and date the Van Usage Policy and provide the COMET Program with a legible copy of your driver's license. Please fax these documents to (303) 497-8491 (The COMET Program).

Note: The minivans are used as transportation to and from the COMET Classroom and can also be used for limited around town use.
